Health Care Facilities Construction and Other Miscellaneous Congressional Earmarks

The program administers Congressional earmarks for local health care projects. In recent years, to ensure passage of annual appropriations bills, Congressional leaders have consolidated several separate appropriations bills into an Omnibus bill, typically containing thousands of earmarks that guarantee broad support.

A rating of Results Not Demonstrated (RND) indicates that a program has not been able to develop acceptable performance goals or collect data to determine whether it is performing.
  • Earmarked projects often serve local interests and do not fulfill national priorities or needs. Earmarks do not necessarily reflect a community's most pressing needs and bypass the local planning priorities. There is some evidence that earmarks in academic settings are less productive than peer-reviewed funding.
  • These projects are not subject to a competitive or merit-based process to ensure higher priorities are funded first. Earmarks are awarded through a Congressional political process, favoring those who have direct access to elected officials or indirect access through lobbyists.
  • Earmarks divert people and associated financial resources from core mission activities of the Health Resources and Services Administration. Projects generally take several years to complete, requiring Ageny resources for an extended period of time. Awardees are often not prepared to absorb the award, requiring additional technical assistance.
